Behavioral Health Group Home Adult Case Manager

Full time/non-exempt/Great Falls

Position Overview: A paraprofessional with knowledge and skills to effectively perform case management duties to the seriously mentally ill who are evaluated to meet both financial and clinical criteria

Job Duties:

  1. Provide assistance to adults with severe disabling mental illness by assisting the client to obtain services, such as income support, transportation, housing, rehabilitation services, and prevocational and vocational services.
  2. Maintain caseload of assigned clients and meet productivity goals.
  3. Complete comprehensive strength-based assessment and reassessment every 90 days.
  4. Develop written treatment plan that coordinates with other services including family supports and outside community services.
  5. Develop crisis plan based on recommendation of the clinician.
  6. Provide outreach and follow-up for individuals who stop treatment.
  7. May develop budget plans with client.

Minimum Qualifications:

  1. Minimum qualifications are a bachelor’s degree in a human services field and at least one year of full-time experience serving people with severe disabling mental illnesses is required.
  2. Other applicable education and experience may be considered.
  3. Travel required.
  4. Must have a personal vehicle, valid driver’s license, and may transport clients
