Employment Type:Full timeShift:Day ShiftDescription:Job DescriptionOncology Outpatient Clinic Nurse:Coordinates the multidisciplinary care of Lung and GI oncology patients. Includes coordinating and participating in the patient care conference, triage and follow up care. Works closely with the Physicians in ensuring that all diagnostic and treatment care is arranged.The Oncology Outpatient Clinic Nurse coordinates the care of the patients through the disease process and acts as a clinical liaison for patients, across the health system and clinical departments. The Oncology Outpatient Clinic Nurse contributes to the assessment of the patient's psychosocial and clinical needs throughout the continuum of care ensuring high-quality care. The Oncology Outpatient Clinic Nurse functions as a liaison between patients, physician practices, the hospital, and the community performing functions that meet the core competencies of oncology navigation. The Oncology Outpatient Clinic Nurse will establish a caring therapeutic relationship with patients and families utilizing effective communication. This person will assess patients for social determinants of health and utilize findings to develop a plan of care and mutual goal setting and be a liaison for the patient during their care and treatment. The position exists to facilitate a timely, integrated, and coordinated approach to the care of patients with a diagnosis of cancer and to provide quality nursing care for patients with cancer-related health issues.Essential Job Functions: Assesses clinical, emotional, spiritual, psychosocial, financial, and other patient needs.Facilitates resolution to patient needs and monitors follow-up.Educates/reinforces education with patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms based on the multidisciplinary care plan.Coordinates discharge planning with the patient's family, social workers, home nursing staff, oncologists, and personal physicians.Works with patients and family members to ensure discharge instructions are understood and followed.Develops and/or coordinates the development of patient education materials, clinical pathways, and quality studies.Coordinates and/or participates in multidisciplinary consultations and/or tumor conferences.Documents recommendations made at multidisciplinary consultations and/or tumor conferences.Patient Care Navigation: Collaborates with the physician in the assessment, planning, and implementation of patient care for cancer patients in the outpatient setting.Conducts patient assessment for learning needs, anxiety level, support systems, coping mechanisms, or social issues o Identifies physical and social barriers to care.Collaborates with resources for the resolution.Coordinates the care of a group of patients including the transition from outpatient to inpatient or inpatient to outpatient; home care/supportive care as needed; scheduling/coordinating scans/tests/labs.Facilitates communication among patients, providers, and agencies.Initiates survivorship plans as appropriate and coordinates with the physicianAssists patient/family navigate the complex issues/decisions as well as work with the multidisciplinary team through orchestrating many tasks that are required prior to and during therapy (central line placements, special testing, obtaining oral chemotherapy from specialty pharmacies, insurance company approval, etc.). Prepares the patient and family for what to expect through each phase of care.Direct Patient Care: Implements orders on the oncology care plan, standard order sets, and given orders (including prescription renewals) and evaluates patient's response to illness and health risks with cancer and treatment, conducts focused physical exams, and collaborates with physicians about findings.Evaluates the patient's response to medical and nursing care.Reports adverse events to the physician, evaluate patient response to interventions, and change plans as needed; collaborate with the physician on complex issues.Collaborates with the physician on the implementation of the oncology care plan.Plans, schedules, and implements nursing care including education, administration of chemotherapy (IV, IM, subcutaneous, oral, intraperitoneal, intrathecal, intralesional, etc.), immunotherapy, supportive care, central catheter care, etc.Triages phone calls to primary patients.Demonstrates and maintains competency, advanced knowledge, and a high skill level of care for oncology/hematology patients throughout the continuum of care.Minimum Education:Required: Associates DegreePreferred: Bachelors DegreeSpecify Degree(s): Nursing.
